Found via Reddit, a tutorial on the Brenizer Method, a process of stitching together lots of telephoto shots to create a large image with narrow DOF.

Must have a go at this with the telephoto lens. Maybe even the kit lens at full extension will do? We’ll see. Will experiment as soon as possible. I’m going to be shooting a good friend’s wedding next year, and shots like this would be fantastic to master.
